Diabetes Self-Management Education/Training Program

The Sun Health Center for Health & Wellbeing is proud to offer a Diabetes Self-Management Education/Training (DSME) Program accredited by the American Association of Diabetes Educators. This program helps individuals with diabetes learn how to manage their disease and be as healthy as possible by focusing on healthy eating, physical activity, self-monitoring, medication management, reducing risks and preventing diabetic complications.

Sun Health’s DSME Program is taught by a certified diabetes educator/registered dietitian and exercise physiologist/health coach.

DSME programs are offered throughout the year at each of the Center’s three locations.

DSME is covered by Medicare and most insurances. A physician referral is required to register for the program. Physician referral and completed paperwork are due 5 business days prior to the class start date. To register or for more information, call (623) 832-WELL (9355).
